Family Support Practitioner

Location: South Western Sydney | Salary: $70,000–$85,000 + Super

As a Family Support Practitioner at Crescent Care, you will work closely with vulnerable families to strengthen relationships, improve parenting capacity, and ensure child safety and wellbeing. Your role will focus on early intervention and connecting families to community resources.

Key Responsibilities:

• Conduct family assessments and develop tailored support plans.
• Provide parenting education and strategies to improve family functioning.
• Facilitate access to healthcare, housing, education, and financial assistance services.
• Work collaboratively with external agencies and service providers.
• Maintain accurate case notes and reporting in line with program guidelines.

Requirements:

• Degree in Social Work, Psychology, or Community Services.
• Minimum 2 years of experience working with vulnerable families.
• Strong case management and crisis intervention skills.
• Excellent communication and cultural competency skills.
• Valid driver’s licence and ability to travel between sites.
