Crescent Care Foundation (ABN 37 656 537 370, NDIS Registration Number 4050154538) is a registered NDIS provider and community services organisation based in Liverpool, New South Wales. We handle personal information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
Information we collect
We only collect what we need to support you well. Depending on how you engage with us, that can include:
- Personal details such as your name and contact information, usually through our forms and enquiries.
- Sensitive information such as health and NDIS details, collected with your consent so we can deliver care.
- Website usage data such as your IP address and browser type, which helps us improve the site.
How we use your information
We use your information to deliver and coordinate your support, send reminders, meet our obligations under the NDIS Practice Standards, and improve the quality of our services. We do not use your information for purposes you would not reasonably expect.
When we share information
We never sell or rent your information. We share it only where necessary, for example with trusted IT providers, with healthcare professionals involved in your care (with your consent), or with regulators such as the NDIS Quality and Safeguards Commission and Services Australia where required.
Cookies, analytics and advertising
With your consent, our website uses cookies and similar technologies from Google Analytics and the Meta pixel (Facebook and Instagram). These help us understand how the site is used, and let us show our services to people who may need them. They are only loaded after you choose to accept them on the banner we show on your first visit.
You can decline at any time by choosing essential only, clearing this site's data in your browser, or disabling cookies through your browser settings. Declining does not affect the services we provide to you.
Your rights
You can ask to access the personal information we hold about you (Australian Privacy Principle 12), ask us to correct it if it is wrong (Australian Privacy Principle 13), and raise a privacy concern with us at any time.
Storing and keeping your information
We take reasonable steps to keep your information secure. We keep it only for as long as we need it or as the law requires, and then we securely destroy or de-identify it.
