Founded in Liverpool
Crescent Care Foundation was founded in Liverpool, in the heart of South West Sydney.
2025
A registered charity
In March 2025 we became a charity registered with the Australian Charities and Not-for-profits Commission.
Today
Growing with our community
We offer NDIS services, allied health, community and settlement support, and SDA and SIL homes, and we are getting ready to offer home care.
Our mission
To empower people with disability and vulnerable community members to live the lives they choose: with dignity, independence, and connection. We believe that with the right support, every person can thrive.
Built in Liverpool. Built for Western Sydney.
Our support workers, coordinators and allied health practitioners work alongside participants and their families. Each person tells us their goals, their culture and what matters to them, and the support is shaped around that.
