Crescent Care Foundation is a registered NDIS provider (Registration No. 4050154538) based in Liverpool, Western Sydney. Founded with a commitment to equity, inclusion, and community empowerment, we deliver high-quality disability support and community services to individuals and families across the Greater Western Sydney region.
Our team of dedicated support workers, coordinators, and allied health professionals works alongside participants and their families to design truly personalised support plans — plans that honour each person’s goals, culture, and lived experience.

Our Commitment to the NDIS
As a registered NDIS provider, Crescent Care is fully committed to the NDIS Code of Conduct and the NDIS Practice Standards. We undergo regular quality audits, and our team receives ongoing training to ensure the safety, wellbeing, and rights of every participant are always protected.
We work with participants across all ages and disability types, providing support across a broad range of NDIS registered categories including Support Coordination, Daily Personal Activities, Community Participation, Household Tasks, Supported Independent Living (SIL), and Specialist Disability Accommodation (SDA).
