Crescent Care is getting ready to offer home care for older people in Liverpool and South West Sydney. Register your interest and we will call you as soon as we can help.
What Support at Home is
Support at Home is the Australian Government program that helps older people keep living at home for longer. It funds services, equipment and changes to your home that help you stay independent.
It replaced the Home Care Packages Program on 1 November 2025. To use it, you first need an aged care assessment through My Aged Care.

How to get home care
Contact My Aged Care
Call My Aged Care or visit their website to check you are eligible and ask for an aged care assessment.
Have your assessment
An assessor talks with you about what you need. If you are approved for Support at Home, funding is allocated to you when it becomes available.
Choose a provider and talk to us
You choose who delivers your care and sign a service agreement with them. Register your interest below and we will call you.
My Aged Care: 1800 200 422, Monday to Friday 8am to 8pm, Saturday 10am to 2pm, or myagedcare.gov.au (opens in a new tab). An interpreter can join these calls, see languages and interpreters.
Who it suits
Older people who want to keep living in their own home and need some help to do it, and the families who look out for them.
Languages and interpreters
- Tell us the language you are most comfortable in, and a professional interpreter can join the conversation.
- If English is not that language, ask for an interpreter at any time through TIS National 131 450.
- For Auslan, eligible Deaf seniors can use the National Sign Language Program for aged care, run by Deaf Connect (opens in a new tab).

Is Crescent Care providing home care now?
Not yet. We are getting ready to offer home care and will contact everyone who registers their interest as soon as we can take on clients. If you need care before then, My Aged Care can explain your options.
Do I need an assessment first?
Yes. Support at Home funding needs an aged care assessment through My Aged Care. You can register your interest with us before or after your assessment.
What will it cost?
Support at Home is funded by the Australian Government, and some people also pay a contribution. My Aged Care can explain what applies to you. We will publish our prices on this website before we start.
