Health & community workers
GPs, hospitals and community services are welcome to refer with consent.

Questions people ask with the form open
Can I refer myself?
Yes. Most of the referrals we receive are people referring themselves or a family member. There is no referral letter, no professional needed in the middle, and no wrong way to fill the form in.
What if I do not know the NDIS details yet?
Send it anyway. Share as much or as little as you have. If a plan number or a funding type is missing we will work it out with you, and if you are not on the NDIS at all we will talk you through what applying involves.
What happens after I press send?
The screen shows a reference number, and the same reference goes to the email address you give. Our intake team reviews the referral and contacts you within one business day.
Do I need the person's permission to refer them?
Yes, if you are referring somebody else. GPs, hospitals and community services are welcome to refer with consent. We will talk to the person themselves before any support starts.
