The fastest way to refer
Four ways in. Pick the one that matches the support you are asking about, and send what you have.
The online referral form
A couple of minutes, and no referral letter needed. Once the referral is saved, the screen shows a reference number you can quote if you follow up.
Make a referralAllied health enquiries
For occupational therapy, psychology, speech pathology, allied health assistant and key worker support. You can attach a report or assessment, and no referral letter is needed to start.
Send an allied health enquirySDA and SIL homes
The homes enquiry asks which home, design category, funding in the plan, preferred area and timeframe. You will get a straight answer on whether one of these homes fits, including if the answer is no.

What coordinators ask us
Before you send the first referral.
Do you need a referral letter?
No. Not for a general referral and not for allied health. Send what you have through the form, or call and we will take the details. If you do have a report or an assessment, the allied health enquiry lets you attach it, which saves a conversation later.
How do I know the referral arrived?
The screen shows a reference number as soon as the referral is saved, and the same reference goes to the email address you give. Quote it if you follow up. Our intake team reviews the referral and contacts you within one business day.
Will you tell me if you cannot take someone?
Yes, and quickly. A coordinator who places a participant somewhere that was not ready does not refer twice, so we would rather say no on day one than leave you waiting. Where we cannot help we will say what we would suggest instead.
How do I hear when a home or a service has room?
Sign up for the quarterly capacity updates further down this page. It is an opt in rather than a referral, it holds a place for nobody, and every email carries an unsubscribe link.

Once a quarter we email referrers about where Crescent Care has capacity and which homes are available, so you are not guessing before you call.
